The reason for this is that promotions up to 1/2 colonel or naval commander are mostly based on merit. Promotions above these ranks are largely thanks to political sponsorship, which becomes more important and prevalent the higher the rank sought. This is how lieutenant colonel Wesley Clark rose to four star general in six years (in peace time!), he was the Clintons GI Joe boytoy.<br>Senior army officers knew this guy was (is) a nut, but to say so would be to contradict the "religion of peace" mantra accepted in Washington DC, and not just by the O, GWB, conscious of his family's financial ties to the House of Saud, also spoke this nonsense. So officers who are responsible avoided their duty and as a result we have a mass murder. You can be sure that many officers are right now busily covering their tracks so that their careers don't suffer any damage from this incident.
